Smackover-Norphlet School District will conduct sports physicals on the morning of April 20, 2023, for students in grades 6-11.
Physicals will be held at Smackover High School. A bus will transport students from Norphlet Middle School to SHS for their physicals.
If the student is not a patient of Smackover Clinic, a fee of $30 (cash only) is due. If you are a patient of Smackover Clinic, there is no fee required.
Students can pick up forms at the front office of both SHS and NMS. Forms and/or money will be due on April 14. No forms or money will be accepted on the day of physicals.
