COVID Vaccine for Students

The Arkansas Department of Health has authorized COVID vaccinations for people 16 years old and older.  Our district has the opportunity to have a vaccine clinic at our school for students 16 years old and older.

Please sign and return the form below to school by Tuesday 4/26/21 if you would like your student to receive the COVID vaccine.

Once this form is returned to the school, the numbers will be added to see if there are enough students who want a vaccine to have a clinic here at school.  If you have any questions, please call the school nurse.
